Schizophrenia and Psychosis Report - A Survey For Individuals

The Schizophrenia Society of Ontario (SSO) is committed to making a positive difference in the lives of individuals, families, and communities affected by schizophrenia and psychotic illnesses. To make sure that the mental health system is truly responsive to the needs of individuals and families impacted by these conditions, the SSO is creating our first ever Report on Schizophrenia and Psychosis in Ontario.
This is where we need your help! Input from individuals living with schizophrenia, psychotic illnesses, and experience of psychosis is not only invaluable, but will serve as the foundation for our report.

The survey will focus on your experience with programs and services currently available through the mainstream mental health system. You will also have opportunities to share your feedback on what other formal or informal supports you found helpful.
The survey takes about 30 minutes to complete. All information gathered through this survey will be kept confidential and anonymous. The survey can be completed online at
